The Health Assistant (HA) works as a member of a multi-disciplinary primary care team providing integrated primary care services to homeless and low-income patients seen at Old Town Clinic. The Care Team is responsible for proactively managing the healthcare needs of the patients on their assigned panel, including behavioral and medical health needs. The HA is responsible for providing support to the Care Team Managers (CTM) and providers on each primary care team. Program: Old Town Clinic is a health clinic providing treatment for acute and chronic health concerns, primary care, mental health support, care for substance use disorders and an on-site pharmacy. Old Town Clinic is adjoined to the Old Town Recovery Center and the Richard Harris Building (8x8). The clinic opened in 1986 and became a part of CCC in 2001.
