About The Position

This position is for a temporary employee working days, focusing on OP Care Management. The specialist will be responsible for establishing trusting relationships with patients and their families, as well as various community partner organizations. They will utilize motivational interviewing techniques to provide individual support and coaching, coordinate and manage closed-loop referrals, and lead/teach health classes. The role also involves providing transportation to necessary health and social services, offering general health and wellness education, identifying and overcoming barriers to connect patients with services, and working cooperatively with other clinical personnel. The specialist will operate Sky Lakes owned motor vehicles safely and professionally and must remain calm under stressful situations. Frequent sitting, standing, and/or driving throughout the shift are required, along with the ability to perform varied activities.

Requirements

  • Possession of valid Oregon driver’s license
  • Excellent driving record
  • Ability to be insured by the hospital’s auto insurance policy
  • Ability to work independently without constant supervision
  • Ability to effectively communicate with patients, medical personnel, community partners and others they may come into contact with, in a pleasant and constructive manner
  • Ability to prioritize workload in a timely manner
  • Ability to take direction from lead, manager or director regarding work activities and procedures
  • Ability to drive safely throughout the geographic area defined by Sky Lakes Medical Center, in all types of weather.
  • Ability to climb a minimum of two flights of stairs.
  • Ability to work with periods of indirect supervision in patient’s place of residence.
  • Ability to withstand constant walking on even and uneven terrain, standing, bending and lifting at least 25 lbs. regularly and 50 lbs. occasionally, push/pull DME equipment, and occasional sitting.
  • Ability to function during periods of stress, and exposure to potential elements in the field and have the requisite visional, auditory and fine motor skills.
  • Frequent sitting, standing, and/or driving throughout the shift required.
  • Must be able to perform varied activities.

Responsibilities

  • Establishing trusting relationships with patients and their families and various community partner organizations.
  • Utilizing motivational interviewing techniques to provide individual support and coaching for patients and their families.

  • Leading, teaching, and facilitating various health classes at the Wellness Center and other community partner sites.
  • Providing transportation to necessary health and social services to qualified beneficiaries.
  • Providing current and appropriate general health and wellness education throughout interaction with patients.
  • Identifying barriers and implementing strategies to overcome barriers in efforts to connect patients to health and social services.
  • Working cooperatively with other clinical personnel assigned to same patient.
  • Operating Sky Lakes owned motor vehicles in a safe and professional manner.
  • Remaining calm under stressful situations.
  • Performing other duties as assigned.
