About The Position

Our Health and Wellness Manager is a multifaceted leadership role responsible for overseeing the daily clinical operations and well-being of residents in our memory care community. This person acts as a crucial link between the residents, their families, and the memory care staff, ensuring a high quality of life and compliance with all regulations.

Requirements

  • Education/certifications in healthcare administration, nursing, or a related field.
  • Managerial experience in a clinical setting.
  • Experience in a senior living or long-term care setting.
  • Exceptional organizational and problem-solving abilities.
  • Strong interpersonal and communication skills to build rapport with residents, families, and staff.
  • A compassionate and empathetic demeanor.

Responsibilities

  • Developing, implementing, and monitoring personalized service plans for residents, including conducting assessments, coordinating with healthcare professionals, and ensuring residents receive necessary services.
  • Creating a resident-centered approach to the planning and delivery of services, supporting residents’ rights to participate in decision-making and treating residents with dignity and respect.
  • Accurately completing and using level of care determination tools to ensure residents receive the care and services they require and desire.
  • Performing assessments on health, functional, and psychosocial status of residents and participating in the development of individualized service plans.
  • Proactively managing care and services for each resident, maximizing the resident’s opportunity to remain in the assisted living environment.
  • Problem-solving as part of the management team to case manage and meet the needs of residents, assessing or evaluating residents with reported changes of condition, and providing team member direction, service planning, and coordination of care.
  • Collaborating with the management team on the move-in of prospective residents, considering the needs of the prospective resident, current community acuity, and staffing levels.
  • Providing in-person pre-move-in assessments, if necessary, and ensuring resident care needs are met by coordinating services with other professionals.
  • Partnering in the investigation and root cause analysis of incidents and collaborating with the management team on the development and implementation of appropriate interventions.
  • Overseeing the documentation and communication of resident care and services, ensuring resident records are completed in a timely manner and maintained according to company policies and state regulations.
  • Managing the health services team, including hiring, training, scheduling, and evaluating staff.
  • Providing on-call support and phone triage for team members’ questions regarding resident care.
  • Providing floor coverage when needed and oversight to the staffing schedule.
  • Designing and implementing programs that promote the physical, emotional, and social well-being of residents.
  • Maintaining open communication with residents and their families, addressing concerns, resolving conflicts, and providing updates on resident status.
  • Providing education and support to residents and families.
  • Acting as an advocate for residents, ensuring their needs and rights are protected.
  • Ensuring the memory care community adheres to all local, state, and federal regulations.
  • Managing budgets, overseeing medication programs, and maintaining detailed resident records.
