About The Position

We are seeking an experienced and compassionate Health & Wellness Director to provide clinical leadership and oversee the delivery of high-quality, resident-centered care within our community. The Health & Wellness Director plays a critical leadership role in coordinating residents' comprehensive health needs, overseeing medication management and care services, developing individualized service plans, and partnering with residents, families, healthcare providers, and team members to promote safety, dignity, and quality of life. This is an opportunity for a healthcare professional who is ready to combine clinical expertise with leadership, advocacy, and relationship-building. As Health & Wellness Director, you will provide hands-on leadership across the community's health and wellness operations. You will assess resident needs, coordinate care, support healthcare transitions, oversee medication processes, and ensure that care plans and documentation remain accurate and current. You will also serve as a resource and mentor to the resident care team, helping staff deliver proactive, compassionate care while maintaining compliance with applicable assisted living regulations and company standards.

Requirements

  • High school diploma or GED.
  • Current nursing license in good standing.
  • Minimum of one year of experience in dementia care and medication administration.
  • Current First Aid/CPR certification upon hire, maintained as required.
  • Experience with Microsoft Windows, Microsoft Office, and internet-based applications.
  • Valid driver's license and access to a private vehicle for business use.
  • Current Arizona fingerprint clearance card for health care institutions, or application within the required timeframe.
  • Successful background screening appropriate for an assisted living environment.
  • Ability to read, write, and speak English fluently.
  • Strong written and verbal communication skills.
  • Effective problem-solving, organization, time-management, and prioritization skills.
  • Ability to work effectively under pressure, manage frequent interruptions, meet deadlines, and communicate professionally with residents, families, employees, healthcare providers, and community leadership.

Responsibilities

  • Oversee supervisory, personal, and directed care services.
  • Assess residents' physical, functional, and psychosocial status and respond appropriately to changes in condition.
  • Develop, implement, and regularly review individualized Resident Service Plans in collaboration with residents, families, healthcare providers, and the care team.
  • Coordinate continuity of care with home health, hospice, and other outside agencies.
  • Conduct preadmission screenings and support the resident admission process.
  • Monitor residents receiving care in hospitals or skilled nursing facilities and help coordinate their return to the community.
  • Lead resident care conferences and collaborate with providers, families, and staff to address care needs and concerns.
  • Advocate for services and resources that support each resident's comprehensive health and wellness needs.
  • Oversee medication ordering, receiving, administration, documentation, and follow-up.
  • Review medical practitioner orders and ensure they are accurately implemented and documented.
  • Coordinate medication changes, pharmacy communications, laboratory results, and provider orders.
  • Conduct routine medication audits and participate in scheduled pharmacy audits.
  • Ensure medications, medical equipment, and supplies are available and appropriately managed.
  • Provide training and education to resident care staff.
  • Assign caregiver responsibilities to support safe, proactive, high-quality resident care.
  • Mentor employees through respectful communication, positive reinforcement, and constructive feedback.
  • Review incident reports and documentation and provide follow-up education as appropriate.
  • Promote a positive, professional, and collaborative workplace culture.
  • Participate in Quality Assurance and Improvement activities.
  • Maintain accurate and current resident records and documentation.
  • Communicate timely changes in resident condition and care with families, representatives, providers, and staff.
  • Participate in safety committee meetings and community functions.
  • Ensure compliance with federal, state, and local assisted living regulations as well as company policies and procedures.
  • Maintain confidentiality of resident, family, and employee information.
  • Respond professionally and promptly to resident, family, and employee concerns.
  • Participate in shared Director on-call responsibilities.
  • Address time-sensitive resident, family, staffing, and medication concerns during on-call periods.
  • Support the community during emergency situations and other circumstances requiring continued operations.
  • Contribute to a positive resident and family experience and support community occupancy and growth initiatives.
