About The Position

The Health and Wellness Coordinator (LPN/LVN) role is for individuals passionate about serving seniors and making a daily difference in their lives within a senior living building. This position is with a Great Place to Work Certified company that offers numerous perks and programs. The role involves assuring the implementation of resident care policies and procedures, supervising and assisting in all resident services, and coordinating ancillary health care providers. It also includes conducting assessments, developing service plans, updating service information, and ensuring timely and accurate resident assessments and charges. A key part of the role is assisting with Social History Assessments and developing life enrichment programming through resident and family meetings, and coordinating with associates and volunteers for implementation and follow-up.

Must be an LVN or LPN

Responsibilities

Assures implementation of policies and procedures relating to Resident care.
Supervises and assists in provision of all Resident services, including the coordination of those provided by independently contracted ancillary health care providers as requested by Residents and families.
Conducts assessments, negotiates service plans, develops service schedules and updates service information as indicates by Resident needs.
Ensures that Residents are appropriately assessed on a timely basis and charged in accordance with those assessments.
Assists in the Social History Assessments and outlines life enrichment programming through these assessments to include meetings with residents and families and implementation and follow up with associates and volunteers on life enrichment programming.
