Health and Wellness Coordinator (LPN/LVN)
About The Position
The Health and Wellness Coordinator, requiring an LPN or LVN, plays a crucial role in resident care within a senior living building. This position ensures the implementation of resident care policies and procedures, supervises and assists in all resident services, and coordinates services provided by external healthcare providers as requested by residents and their families. The role involves conducting assessments, developing service plans and schedules, and updating service information based on resident needs. Additionally, the coordinator is responsible for timely resident assessments and accurate billing, assisting with Social History Assessments, and developing life enrichment programming through these assessments, including resident/family meetings and follow-up with associates and volunteers.

Responsibilities
- Assures implementation of policies and procedures relating to Resident care.
- Supervises and assists in provision of all Resident services, including the coordination of those provided by independently contracted ancillary health care providers as requested by Residents and families.
- Conducts assessments, negotiates service plans, develops service schedules and updates service information as indicates by Resident needs.
- Ensures that Residents are appropriately assessed on a timely basis and charged in accordance with those assessments.
- Assists in the Social History Assessments and outlines life enrichment programming through these assessments to include meetings with residents and families and implementation and follow up with associates and volunteers on life enrichment programming.
