About The Position

The Healing To Wellness Court Case Manager - Judicial will report to the Presiding District Court Judge and manage the efficient delivery of case management duties for the Healing to Wellness Court staff and judges, observing all relevant procedures. This position offers a hybrid work possibility after the first 90 days. The primary tasks involve conducting client investigations, intake, orientation, and assessments to determine treatment and recovery needs. The role includes developing, implementing, and monitoring individualized treatment and wellness plans, coordinating chemical dependency evaluations, and facilitating counseling sessions. The Case Manager will provide substance abuse education, monitor client progress, collaborate with clinical staff and families, and connect clients with community resources. Additionally, the role involves organizing activities that support holistic recovery, developing relapse prevention and aftercare plans, and maintaining case management documentation. Other assigned duties may also be part of this role.

Requirements

Bachelor's degree in human services/mental health, criminal justice, social work, or another related field or four (4) years of directly related experience.
Licensed Addictions Counselor or Certified Addiction Counselor.
Alcohol and Drug Counselor trainee status or ability to obtain within the 90-day probation period.
Knowledge of individual case planning, case management, and working with individuals in a helping capacity.
Excellent case and file management skills.
Knowledge of available Choctaw Nation and community resources.
Knowledge of Healing to Wellness Court goals and strategies, violence and addiction reduction initiatives.
Demonstrate a high level of cultural competence, sensitivity, and responsiveness.
Must have a valid driver's license.
Must be able to pass a criminal background check.

Responsibilities

Conduct client investigation, intake, orientation, and assessments to determine treatment and recovery needs.
Develop, implement, and monitor individualized treatment and wellness plans.
Coordinate chemical dependency evaluations and appropriate treatment services.
Facilitate individual and group counseling sessions aligned with treatment goals.
Provide substance abuse education and promote recovery principles, including Wellbriety and Choctaw recovery programs.
Monitor client participation, compliance, and progress throughout treatment.
Collaborate with clinical staff, supervisors, families, and support systems to support client success.
Coordinate referrals and connect clients with community resources and support services.
Organize educational, cultural, spiritual, and recreational activities that support holistic recovery.
Develop relapse prevention, aftercare, discharge plans, and maintain required case management documentation and records.
Perform other duties and responsibilities as may be assigned.
Provide client orientation, describing the nature and goals of the program, procedures, and daily schedule.
Assess client problems and needs as determined by an American Society of Addiction Medicine (ASAM) assessment.
Develop a written treatment plan for each assigned client with established therapeutic goals.
Provide personal and group counseling therapy to address the treatment plan goals.
Meet with supervisors and other clinical staff to review all the clients' treatment progress or lack thereof and determine suitable courses of action to follow.
Provide opportunities for clients to gain an understanding of the need for involvement in-step principles and CHOCTAW NATION phases of recovery and group support.
Conduct daily substance and other addiction education about the nature of alcohol and drug dependence and other addictions.
Perform case management and record keeping coordinating services to referral agencies and resource persons in meeting treatment plan objectives.
Develop relapse prevention and aftercare plans for clients to follow upon completion of primary treatment.
Prepare discharge summaries including client diagnosis, course of treatment, progress on treatment plans, reason for discharge and aftercare plans.
Provide chemical dependency evaluations for adults who are in possible need of inpatient, day treatment or outpatient treatment for alcohol or other drug disorders.
