About The Position
Youth Haven Services is expanding its Intensive In-Home (IIH) program and is seeking dedicated Qualified Professionals to join their team. This role focuses on providing direct support, skill building, case coordination, and therapeutic interventions for youth with behavioral and mental health needs, all guided by their Person-Centered Plans. Services are delivered in the home and community, following YHS and state guidelines. Youth Haven Services is a comprehensive mental health agency specializing in serving children, adolescents, and families.
Requirements
- Bachelor’s degree in a human services field (required)
- Two years of documented experience working with youth/adolescents with mental health needs
- Strong documentation and organizational skills, including ability to maintain logs and clinical records
- Ability to teach skill-building activities and support clients in home and community settings
- Reliable transportation, valid driver’s license, and active auto insurance
- Ability to engage youth and families in a supportive, therapeutic manner
- Strong communication skills and the ability to collaborate with diverse populations
- Ability to follow state service definitions and program requirements
- Ethical, professional, and reliable work habits
- Strong time-management and problem-solving skills
Responsibilities
- Complete initial intakes, Person-Centered Plans, and submit authorizations/reauthorizations
- Maintain accurate, up-to-date clinical records, including consents, intake packets, and required documentation
- Complete and submit NC TOPPS, pre/post outcomes, discharge SARs, and other program paperwork
- Coordinate care, scheduling, and person-centered planning for assigned clients
- Assist in identifying placements for clients and completing required placement documentation
- Monitor caseloads, track authorizations, and verify insurance monthly
- Provide strength-based, recovery-focused interventions
- Support skill building, coping strategies, and community functioning
- Perform other duties as required
