About The Position

The HAB Access Specialist is a field-based member of the Home Assist Health Habilitation team. This role exists to deliver individualized, person-centered habilitation services to members under Arizona DDD/AHCCCS programs — helping them acquire skills, build independence, and achieve the goals outlined in their Person-Centered Support Plan (PCSP). This is a direct service role. The HAB Access Specialist's primary job is to show up for members, implement their goals with fidelity, and document sessions accurately. This role requires a driver who is flexible, reliable, and genuinely committed to person-centered care.

Requirements

  • High School Diploma or GED
  • Valid Arizona driver's license and reliable personal vehicle
  • Arizona Level I Fingerprint Clearance Card (or willingness to obtain before start date)
  • CPR and First Aid certifications (or willingness to obtain)
  • Article 9 training completed (or willingness to complete)
  • Genuine commitment to person-centered care and disability rights
  • Willingness to work flexible hours and cover shifts across service areas
  • Minimum 1 year of experience working with individuals with intellectual or developmental disabilities
  • Familiarity with Arizona DDD/HCBS programs and habilitation services
  • Experience electronic documentation platforms Like AxisCare

Nice To Haves

  • Knowledge of ABA principles or habilitation goal implementation
  • Past experience as a DSP, RBT, or BHT

Responsibilities

  • Deliver individualized HAB sessions per each member's PCSP goals — teaching skills, building independence, and fading support as members progress
  • Apply person-centered thinking in every interaction — honor member voice, choice, and dignity in all activities
  • Facilitate community-based activities, outings, and skill-building experiences that align with member goals
  • Recognize and report changes in member condition, behavior, or environment to the HAB Supervisor promptly
  • Build consistent, trusting relationships with members and their families
  • Clock in and out for every session using existing systems — no exceptions
  • Complete session notes using existing systems for every visit documenting member participation, progress toward goals, and any observations
  • Submit documentation on the same day as service delivery
  • Be available to cover shifts across assigned service areas including Central Phoenix, Mesa, Tempe and Scottsdale and surrounding communities
  • Respond to shift coverage needs as directed by the HAB Manager
  • Maintain reliable personal transportation and a valid Arizona driver's license — Paid Travel Time reimbursement provided per HAH policy
