The Department of Economic Security (DES) is seeking an experienced and highly motivated individual to join their team as a Habilitation Nurse II. This role is responsible for understanding the physical healthcare needs and coordinating care for members with intellectual/developmental disabilities (I/DD) who require skilled nursing care. The position involves specialized nursing case management, completing required documentation and reporting, obtaining and renewing physician's orders, acting as a consultant to individuals, families, and staff, and participating as a member of interdisciplinary teams. The Habilitation Nurse II will also represent the medical component during quarterly Person Centered Support Plan (PCSP) meetings. Flexibility in schedule is desired due to the 24-hour residential settings and periodic monitoring requirements. The role may require working weekends, holidays, nights, and overtime as necessary. DDD provides individuals with developmental disabilities, and their families, with flexible, high-quality, and member-driven services and supports that offer opportunities for independent decision-making and community engagement. Applicants cannot be licensed or certified by the DDD Office of Licensing Certification and Regulation, cannot have a principal interest in or have a spouse or close family with principal interest in a DDD Qualified Vendor Agreement, and cannot be employed or subcontracted by a DDD Qualified Vendor.
