We are a non-profit organization specializing in service delivery to individuals with an intellectual/developmental disability (IDD) diagnosis and/or mental health diagnosis. We work with Individuals, families, long term care facilities, providers, and other community organizations to meet the needs through person-centered planning. We are a small team of super-organized, passionate, advocates tasked with providing essential support to adults in long-term care facilities with IDD. Our charge includes ensuring each person gets the services available to them and ensure living in a long-term care facility is their choice. We are dedicated to making a difference in the lives of our Individuals. Each team member is trained through web-based trainings provided by Texas Health and Human Services. This position receives one-on-one training in the field and ongoing support from the entire team. We split our work between time spent in long-term care facilities to routinely meet with our Individuals and time spent remotely managing documentation and administrative tasks. PASRR (Preadmission Screening and Resident Review) is a federally required process to ensure that individuals with IDD and/or mental illness are not inappropriately placed in a long-term care facility. Many people can live safely in the community with proper support. However, those that choose to stay in a long term care facility are supported and monitored by the habilitation coordinator.
