The Habilitation Coordinator manages services for participants in one of the following settings: nursing facility or community. In the nursing facility, the Habilitation Coordinator is responsible for PASRR duties. In community settings, the Habilitation Coordinator is responsible for performing enhanced community coordination (ECC) duties for individuals transitioning or diverting from institutional care. The Habilitation Coordinator is responsible for assessment, service planning, monitoring, advocacy and crisis prevention and management for assigned caseload of individuals with IDD. Provision of Medicaid billable Habilitation Coordination activities for individuals in a nursing facility. Provision of Medicaid billable targeted case management activities for individuals in a nursing facility transitioning to the community. On call response to PASRR Evaluation alerts within 72 hours. Compliance with applicable local, state and federal rules and regulations. Adherence to and promotion of the Texana Compliance Policy. Travel within 6 county region.
