About The Position

Guardian Conservator position within the Supported Decision Making & Guardianship Options team. LSSMN is the largest professional provider of Guardianship and Conservatorship in the state of Minnesota. The role involves carrying out duties of court-appointed Guardian/Conservator as set forth in MN Statutes, providing financial management, and ensuring care for vulnerable adults in the Guardianship Options Program. The position typically covers a geographic region of the state with a caseload of 35-42 individuals.

Requirements

Two years’ experience with similar client populations and needs
Valid driver’s license
Previous lived experience in making clear and unbiased thinking and decision-making skills.
Ability to exercise sound judgment regarding best interest of client, major financial transactions, and life & death situations.
Extreme detail attention to communications, accurate and timely documentation, and billings.
Must be cleared by a Bureau of Criminal Affairs (BCA) background study and have a drivers’ license; a Motor Vehicle Check (MVR) is required.

Must be obtained after one year of employment: Employer-sponsored accreditation through the Center for Guardianship Certification (CGC)

Responsibilities

Advocate for individuals for least restrictive life choices that enable them to live full and abundant lives.
Work with individuals and families in the development and service delivery of supported decision-making.
Work closely with courts, including annual reports.
Supervise and monitor client personal and health care. This can include authorizing medical, therapy, treatment, support, vocational, and other services.
Sign legal documents such as consents, admissions, and lease agreements.
Work as a member of support team to ensure appropriate living environment and services.
Travel to visit clients at least monthly.
Provide substitute decision-making regarding medical emergencies and other sudden changes in client's situation.
Be responsible for the financial management and oversight of individuals assigned to them.
Work with financial team to oversee financial assets.
Be a member of an on-call schedule for client emergencies.
