About The Position

Under supervision, the clinician provides a wide range of psychosocial evaluation/assessment, diagnostic, counseling therapy, crisis intervention, group facilitation, and/or case management services in an integrated, multi-disciplinary primary care setting, located in a community health center. The role involves working collaboratively with other mental health and medical providers and trainees engaged in related therapeutic/patient care activities.

Requirements

  • Master's degree in Counseling or Social Work
  • Licensed Independent Clinical Social Worker (LICSW), Licensed Mental Health Counselor (LMHC) or equivalent or licensure pending, as documented by temporary license.
  • Ability to make administrative/procedural decisions and judgments.
  • Ability to observe, assess, and record symptoms, reactions, and progress.
  • Knowledge and understanding of clinical social work and mental health counseling including competencies, principles, methods, procedures, ethics, and standards.
  • Interviewing and psychological/developmental evaluation skills.
  • Knowledge of clinical operations and compliance guidelines.
  • Familiarity as well as collaboration with community mental health resources.
  • Ability to maintain emotional stability during crisis or emergent situations.
  • Strong interpersonal skills including the ability to engage and redirect group members as needed and the ability to work effectively with a wide range of constituencies in a diverse community
  • Strong verbal and written communication skills including creating, composing, and editing written materials.
  • Knowledge of crisis intervention techniques and ability to de-escalate in stressful situations.
  • Ability to evaluate the progress of therapeutic interventions and adjust treatment plans as needed.
  • Knowledge of legal and ethical issues related to patients' rights in MA.
  • Ability to lead and train staff and/or students, as needed.

Responsibilities

  • Provides individual, group, and family therapy to clients as appropriate to the position; prepares treatment plans, and case management including discharge planning; provides therapeutic crisis stabilization services as required.
  • Collects data relating to patients through interview, case history, psychological assessments, and/or observational techniques; evaluates data to identify root causes of problems and determines proper the appropriate therapeutic approach or referral to other providers, as appropriate.
  • Consults with supervisor, or other behavioral health and medical staff, as appropriate, in the performance of therapeutic and/or case management services; refers clients to appropriate service agencies based on medical necessity.
  • Provides and/or arranges for therapeutic interventions as appropriate for patients or clients requiring crisis stabilization or level of care change, specifically for those with acute Mental Health and/or active substance use disorder.
  • Refers clients to appropriate community/social service agencies for financial assistance and/or other necessary services.
  • Provide outcome measurements as required to evaluate treatment progress and adjust treatment plan as needed.
  • Establishes and maintains all aspects of client case file including clinical documentation beginning at intake and throughout client discharge, including closing treatment plans for clients completing day treatment and/or transitioning to outpatient behavioral health services.
  • Maintains relationships with referral partners; collaborate with outside treatment providers agencies in relation to patient/client records, rights, and responsibilities while engaged in day treatment.
  • Works alongside community outreach and education activities, as appropriate to the position; may serve as a liaison and representative to community organizations and schools.
  • If appropriate, provide clinical supervision to Master’s level therapists and/or graduate social work students/interns, and/or education or consulting services to other patient care professionals.
  • As appropriate to the position, perform various administrative functions such as group preparation, assist with scheduling, chart audits for quality improvement, utilization review, and outreach to referral sources, etc.
  • Participate in weekly clinical supervision and twice monthly group supervision, attend staff meetings, morning rounds, and multi-disciplinary team meetings as appropriate to the role.
  • Provide psychoeducation, consultation, and in-service training to staff, if/when required.
  • Demonstrate a comprehensive understanding and commitment to the health center Mission, Values & Standards.
  • Provide a description of Day Treatment to new clients.
  • Perform miscellaneous job-related duties as required. Demonstrate understanding and commitment of the health center mission.
