About The Position

CSpring partners with government and healthcare organizations to solve complex problems through data, technology, and people. As a Regional Grant Coordinator supporting Indiana’s GROW Regional Grant Program, you will help turn a historic investment in rural health into coordinated, accountable action across multiple regions, strengthening healthcare access, prevention, and long-term system resilience. Your work will have a direct connection to rural communities across Indiana. GROW represents approximately $600 million in multi-year investment across 64 fully rural counties and nine partially rural counties, giving you the opportunity to support work with meaningful, statewide reach. You will help local priorities move from plans to implementation. Each region has developed a unified strategy based on local needs, community priorities, and IDOH data, and you will help regional partners keep those strategies moving forward. You will work across healthcare, public health, and community organizations. You will collaborate with hospitals, clinics, EMS providers, community organizations, Technical Assistance Providers, and state partners while helping everyone work from clear and consistent expectations. You will have independence while staying connected to a larger team. This is a remote, field-based contract role supporting multiple regions, with close connection to CSpring, IDOH, regional coalitions, the Regional Committee and Media Liaison, and statewide Technical Assistance Providers. You will help build systems that last beyond individual projects. From reporting practices to grants-management tools and shared templates, your work will strengthen how regional partners manage funding, measure results, and sustain rural health improvements over time.

Requirements

Bachelor’s degree in public health, health administration, public policy, business administration, or a related field.
5 years of experience in grant management, contract oversight, program administration, or related work.
3 years of experience working with healthcare organizations, rural communities, or public health systems.
2 years of experience demonstrating a strong understanding of compliance, budgeting, and reporting requirements for state or federal grants.
Ability to manage multiple projects simultaneously with strong attention to detail.
Excellent communication, facilitation, and relationship-building skills.
Willingness and ability to travel regularly within different regions of Indiana.

Nice To Haves

Experience with CMS-funded programs or federal cooperative agreements.
Familiarity with Indiana’s rural health landscape, including hospitals, FQHCs, EMS, and community-based organizations.
Experience collaborating with statewide associations or multi-stakeholder coalitions.
Knowledge of rural health delivery, care coordination, or health system transformation.

Responsibilities

Serve as an operational link between IDOH and regional partners, helping multiple regions manage grants effectively, meet state and federal expectations, and maintain progress toward GROW goals.
Serve as a supportive regional contract manager for organizations receiving Rural Health Transformation Program funding, helping partners understand and meet grant and contract requirements.
Monitor compliance with state and federal requirements, including allowable expenditures, budget adherence, documentation standards, milestones, deliverables, and timelines.
Maintain audit-ready grant documentation and support consistent use of state-approved templates, tools, reporting systems, and grants-management practices.
Identify performance risks, compliance concerns, budget issues, or implementation barriers early and escalate issues to IDOH when appropriate.
Collaborate closely with statewide Technical Assistance Providers—IHA, IPHCA, and IRHA—to provide coordinated and consistent support to regional coalitions.
Participate in regional consortium meetings, workgroups, and planning sessions, and serve as a convener for Regional Committee meetings.
Coordinate regional data collection, performance tracking, outcome measurement, and reporting in alignment with CMS Rural Health Transformation Program requirements.
Help regional partners understand CMS documentation, metric, narrative, and reporting expectations, and support corrective action planning when progress falls behind expectations.
Serve as a day-to-day liaison among IDOH, regional coalitions, hospitals, clinics, EMS providers, community organizations, the Regional Committee and Media Liaison, and other partners.
Help partners interpret state guidance, solve implementation challenges, share effective practices across regions, and use consistent statewide tools and evaluation approaches.
