About The Position

The RHTP Grant Program Manager is responsible for the overall administration, coordination, monitoring, and reporting of the Regional Health Transformation Program (RHTP) grant and associated initiatives. This position serves as the central point of accountability for ensuring compliance with grant requirements, achievement of program objectives, and successful implementation of funded projects. The role oversees grant deliverables, budgets, contracts, reporting requirements, performance metrics, and stakeholder engagement while ensuring the effective stewardship of grant funds and program resources.

Requirements

  • Experience managing state, federal, foundation, or healthcare transformation grants.
  • Knowledge of grant compliance, performance measurement, and healthcare regulatory requirements.

  • Proficiency with Microsoft Office Suite, including Word, Excel, PowerPoint, Outlook, and Teams.

Responsibilities

  • Serve as the primary organizational administrator of the Regional Health Transformation Program (RHTP) grant.
  • Manage grant activities in accordance with funding requirements, contractual obligations, regulatory standards, and organizational priorities.
  • Maintain comprehensive knowledge of grant guidelines, performance expectations, reporting requirements, and compliance standards.
  • Ensure all program deliverables, milestones, and objectives are achieved within established timelines.
  • Monitor implementation progress and identify potential risks, barriers, and corrective actions.
  • Maintain accurate grant documentation, records, and audit-ready files.
  • Coordinate implementation of RHTP-funded initiatives across multiple departments, healthcare partners, and community organizations.
  • Develop and maintain work plans, project timelines, implementation schedules, and performance tracking systems.
  • Track and monitor key performance indicators, outcomes, and program deliverables.
  • Develop systems for collecting, validating, and analyzing program data.
  • Monitor project performance, population health measures, utilization trends, and outcome metrics.
  • Prepare dashboards, scorecards, and reports that communicate program performance to leadership and stakeholders.
  • Identify opportunities for improvement based on data analysis and program evaluation results.
  • Ensure data integrity and compliance with reporting requirements.
  • Prepare and submit required grant reports, progress updates, financial summaries, and supporting documentation.
  • Coordinate collection of information from internal departments and external partners to support reporting activities.
  • Monitor compliance with grant regulations, funding requirements, and contractual obligations.
  • Support audits, monitoring visits, evaluations, and compliance reviews.
  • Ensure all reporting deadlines are met accurately and timely.
  • Maintain records demonstrating program accomplishments, outcomes, and fiscal accountability.
  • Collaborate with finance staff to develop, monitor, and manage grant budgets.
  • Track expenditures and ensure appropriate use of grant funding.
  • Monitor budget performance and identify variances requiring corrective action.
  • Review invoices, contracts, and funding allocations associated with grant activities.
  • Assist in financial reporting and budget forecasting.
  • Support sustainability planning and identification of future funding opportunities.
  • Serve as a liaison between funding agencies and organizational leadership.
  • Prepare presentations, reports, and executive summaries for internal and external audiences.
  • Present program updates to leadership teams, boards, committees, and community partners.
  • Facilitate meetings and manage complex projects involving multiple stakeholders.
  • Promote transparency, accountability, and collaboration throughout the grant lifecycle.
  • Serve as a subject matter resource regarding RHTP initiatives and grant requirements.
