SNF Graduate Practical Nurse (GPN) - Full Time -

Performs the primary functions of an LPN in assisting with assessment, planning, implementing and evaluating the care of all patients, regardless of age, during his/her shift. Responsible for meeting the established standards of skilled nursing practice. Professional Graduate Practical Nurse collaborates with the care team, participates in interdisciplinary care, and advocates for patients.
Requirements
- Graduate of a Nursing Program
- Licensed Practical Nurse
- As required for your care area. May include: CPR/BLS, ACLS, PALS, Specialty Certification recommended

Responsibilities
- Demonstrates time management, critical thinking, delegation, and prioritization.
- Gathers data, monitors, records, and reports changes in patient's condition.
- Advocates for patient.
- Practices ethically and professionally within their scope.
- Maintains clinical competence specific to their care area including organization required education within stated timeframe.
- Coordinates care with interdisciplinary team.
- Communicates effectively.
- Documents appropriately and thoroughly in EMR.
- Promotes EBP (evidence-based practice) and best practices related to patient care.
- Participates in unit councils and staff meetings.
