Performs the primary functions of an LPN in assisting with assessment, planning, implementing and evaluating the care of all patients, regardless of age, during his/her shift. Responsible for meeting the established standards of skilled nursing practice. Professional Graduate Practical Nurse collaborates with the care team, participates in interdisciplinary care, and advocates for patients.

Requirements

  • Graduate of a Nursing Program
  • Licensed Practical Nurse
  • As required for your care area. May include: CPR/BLS, ACLS, PALS, Specialty Certification recommended

  • Prior patient care and customer service experience preferred

Responsibilities

  • Demonstrates time management, critical thinking, delegation, and prioritization.
  • Gathers data, monitors, records, and reports changes in patient's condition.
  • Advocates for patient.
  • Practices ethically and professionally within their scope.
  • Maintains clinical competence specific to their care area including organization required education within stated timeframe.
  • Coordinates care with interdisciplinary team.
  • Communicates effectively.
  • Documents appropriately and thoroughly in EMR.
  • Promotes EBP (evidence-based practice) and best practices related to patient care.
  • Participates in unit councils and staff meetings.
