About The Position

A Graduate Nurse is a nurse who has graduated from an approved program by the State Board of Nursing, but has not yet completed the examination for licensure. This role is responsible for the complete nursing care of a group of patients under the direct supervision of a Registered Nurse. The Graduate Nurse will be enrolled in a twelve-month Nurse Residency Program, which includes orientation, interactive classroom education, and independent self-directed learning. The program also offers pairing with a nurse mentor for continued one-on-one support, designed to assist newly licensed nurses in their transition from student to clinical practitioner.

Requirements

Graduate of an accredited Nursing Program required.

Strong ability to communicate complex and/or controversial topics and concepts to a wide and diverse audience.
Current licensure to practice under a Temporary Practice Permit as a Graduate Registered Nurse in the State of Pennsylvania required.
Current Basic Life Support (BLS) approved curriculum of the American Heart Association (CPR and AED) program required.
Act 33 with renewal.
Act 34 with renewal.
Act 73 FBI Clearance with renewal.
Graduate nurse must successfully complete licensure exam within six months of graduation.

Responsibilities

Ensures that patient assessment and direct/indirect patient care are in accordance with the PA Nurse Practice Act, hospital policies, procedures, and protocols, and other professional standards of care.
Collects pertinent health assessment data on admission and ongoing, focusing on physical, psychosocial, age, cultural, and spiritual needs.
Documents initial nursing assessments within established timeframes.
Implements, evaluates, and revises the individualized plan of treatment and care.
Reassesses patient condition and takes appropriate action, including pain management.
Involves patient/family and other healthcare providers as appropriate.
Ensures completeness, accuracy, and timeliness of documentation.
Develops and coordinates patient education to promote health, foster self-care, and improve outcomes.
Assesses patient readiness to learn, identifies educational barriers, and tailors educational processes.
Provides and reviews written discharge instructions, demonstrating the inter-relationship between education, discharge planning, and continuity of care.
Maintains collaborative relationships with physicians and other healthcare providers to achieve desired patient outcomes.
Communicates pertinent information for interdisciplinary patient care conferences, shift reports, and transfers.
Takes action to proactively identify and resolve patient needs.
Participates in unit performance improvement activities to enhance patient care quality and customer service.
Assists in data collection for performance improvement and recommends modifications.
Demonstrates leadership by utilizing concepts of professional nursing practice.
Demonstrates problem-solving skills using critical thinking and seeking assistance when appropriate.
Delegates patient care appropriately and maintains accountability for actions.
Acts as a patient advocate.
Assists in maintaining organizational/departmental goals.
Assists in the department's compliance with state, The Joint Commission (TJC), OSHA, and other regulatory agencies.
Participates in continuous survey readiness for inspections and surveys.
Participates in the non-punitive medication reporting system.
Ensures accountability for narcotics per regulatory/organizational policies.
Adheres to infection control processes, including standard precautions.
Provides for patient safety in compliance with hospital and nursing department policies.
Utilizes two forms of patient identification before implementing patient care.
Involves the patient and/or family in patient safety standards and processes.
Minimizes the use of patient restraints and monitors per policy if warranted.
Assumes appropriate team role during code situations or mock codes.
Responds effectively during emergency situations, including internal and external disasters.
Assists in maintaining a safe and clean environment for patients, visitors, and staff.
Administers IV push medications, IV fluids, and blood after successful completion of IV skills lab and competency validation.
Performs other related duties as assigned.
