The Bureau of Chronic Disease Prevention works to prevent and reduce the impact of chronic diseases and disabling conditions by addressing associated modifiable risk factors. Chronic diseases including heart disease, stroke, cancer, diabetes, and asthma are among the most common, costly, and preventable of all health problems in Florida and in the United States. The Bureau develops, implements, and evaluates health promotion activities, primary and secondary prevention services, and community-based health interventions across the state. The Bureau partners with state and local agencies, health systems, and community partners to encourage best practices statewide, ensuring the inclusion of persons with disabilities in all program activities. These partnerships ensure coordination and collaboration among and between health systems and providers to stimulate prevention and intervention activities to reduce the morbidity and mortality from chronic diseases and support efficient and effective delivery of health care. This is a professional position under the direct supervision of the Health Systems Section Administrator in the Bureau of Chronic Disease Prevention. This is a highly responsible position that involves advanced skills in planning, implementing, monitoring, evaluating, and reporting on activities designed to reduce the burden of chronic disease statewide, specifically asthma. Duties include, but are not limited to the following:Build relationships with program staff, county health departments, and other internal and external partners to implement effective asthma education and environmental intervention programs.Oversee the development and implementation of a program evaluation plan.Oversee epidemiological analyses of program data and data dissemination to local, state, and federal partners.Convene and facilitate the work of the Florida Asthma Coalition.Oversee the dissemination of training and educational materials through the program’s website, webinars, or in-person meetings.Serve as the primary contact with the Centers for Disease Control and Prevention (CDC) for the awarded cooperative agreement, including managing the budget and grant reporting and overseeing the contracts established under this program. Participate in educational opportunities to enhance professional development. Perform other duties and responsibilities as assigned or required.
