Responsibilities
- Assist patients with daily living activities, such as bathing, dressing, and grooming, as needed
- Provide mobility and ambulation assistance, including transferring patients from bed to chair and walking with them
- Assist with personal care, including toileting, incontinence care, and feeding
- Monitor and report changes in patient condition, including vital signs and behavioral changes
- Maintain accurate records and documentation of patient care and treatments
- Collaborate with interdisciplinary care teams to develop and implement individualized care plans
- Provide support and companionship to residents, promoting a sense of community and socialization
