Geriatric Nurse Practitioner
About The Position
Dignity Health Medical Group - Arizona is seeking a full-time Geriatric Nurse Practitioner to join our busy department in Prescott, Arizona. This position will have an Alzheimer’s and Dementia Care focus. The role involves managing a project volume of 250 patients and seeing office patients, with some off-site home visits in the surrounding area. The typical work week offers the option to work 4 or 5 days per week, involving work in a clinic setting as well as off-site visits throughout the community to accommodate patient needs. A typical patient visit load includes 2 new consults per day and follow-up visits.
Requirements
- Current Board Certification
- Valid medical license
- Valid DEA certificate
- 2-5 years experience in geriatrics with focus on cognitive diseases preferred
Responsibilities
- Provide Alzheimer’s and Dementia Care

- Conduct off-site home visits in the surrounding area
