About The Position
Total Life is a telehealth company dedicated to older adults, offering behavioral health, geriatric care, and longevity services. They are launching a new Longevity Program focused on healthy aging, which includes comprehensive geriatric assessment, chronic disease management, cognitive health, and weight management with GLP-1 therapy. The Geriatric Nurse Practitioner will deliver initial longevity assessments and ongoing medical follow-ups to older adults via telehealth, working alongside the CMO and receiving full training and clinical support.
Requirements
- Active, unrestricted NP license in New Jersey
- Board certification as an Adult-Gerontology NP (AGNP/AGPCNP) or Family NP with geriatric focus, training, or demonstrated interest in aging
- Active DEA registration
- Experience with Medicare documentation and E/M coding
- Comfort with telehealth and EHR documentation
- Reliable high-speed internet and a private, HIPAA-compliant workspace
Nice To Haves
- Experience with comprehensive geriatric assessment
- Familiarity with cognitive screening instruments (Mini-Cog, MoCA)
- GLP-1 prescribing experience
- Multi-state licensure
Responsibilities
- Conduct comprehensive initial longevity assessments (60 min) covering functional status, cognition, nutrition, mobility, and fall risk
- Provide medical follow-up visits for chronic disease management and ongoing care
- Perform cognitive screening and staging; escalate for further assessment where indicated
- Prescribe and manage GLP-1 therapy for appropriate patients
- Conduct advance care planning conversations
- Document in our EHR to support accurate Medicare billing
