About The Position

We are seeking a Geriatric Medicine Physician for a Locum Tenens position in Kalispell, MT. This role involves both outpatient and inpatient services, primarily focusing on a Skilled Nursing Facility/Long-Term Care setting. The physician will manage a patient panel of 12-14 patients, working 3 days a week (Tues-Thurs) from 7 am to 5 pm. This is a clinical-only coverage role, ongoing from ASAP, to provide supplemental support. The position requires rounding and includes collaboration with 1 APP. Responsibilities encompass routine medical evaluations, chronic disease management, annual wellness assessments, and specialized care for conditions like dementia and frailty. The physician will also oversee palliative care, review and approve SNF admissions with comprehensive H&P exams, conduct compliance visits, and manage acute changes in condition. Medication management, including safety and pharmacist recommendations, is a key duty. Participation in interdisciplinary care conferences, family meetings, and goals-of-care discussions is expected, along with completing all necessary documentation. The facility has 99 beds and uses Meditech EMR. Hospital privileges are required, with temporary privileges available.

Requirements

  • Active MT License or IMLC
  • BC/BE

Responsibilities

  • Routine medical evals and chronic disease management
  • Annual wellness and preventive care assessments
  • Management of dementia, frailty, and other complex chronic conditions
  • Oversight of palliative and comfort-focused care plans when appropriate
  • Review and approve admissions to SNF, incl completing comprehensive H&P exams.
  • Conduct physician compliance and recertification visits
  • Provide medical care for long-term care residents
  • Develop, review, and update individualized care plans
  • Manage acute changes in condition, evaluate new symptoms
  • Prescribe, monitor, and adjust medications while reviewing consultant pharmacist recommendations and ensuring medication safety.
  • Participate in interdisciplinary care conferences, family meetings, and goals-of-care discussions as appropriate
  • Complete required documentation, physician orders, certifications
