About The Position

Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Geriatric Case Manager in Naples, Florida. This is a community-based case management role serving older adults and their families, providing both short-term and long-term case management services. The role involves developing individualized care plans, coordinating community resources, conducting home visits, and maintaining ongoing client communication to help seniors remain independent and safely age in place. The position offers strong autonomy with support from a dedicated team.

Requirements

• Bachelor's Degree in Social Work REQUIRED
• Case Management Experience REQUIRED
• Comfortable Conducting Home Visits REQUIRED
• Strong Communication and Organizational Skills REQUIRED
• Ability to Develop Individualized Care Plans REQUIRED
• Compassionate and Client-Focused Approach REQUIRED

Nice To Haves

• Master's Degree in Social Work Preferred
• Experience Working with Older Adults Preferred
