About The Position
Heartline Staffing, in partnership with Purple Cow Recruiting, is hiring a Geriatric Case Manager in Naples, Florida. This role involves providing short- and long-term case management services for older adults living in the community. The Geriatric Case Manager will develop individualized care plans to promote independence and quality of life, conduct home visits and ongoing client assessments, and coordinate community resources while communicating with families and healthcare providers. This is a mission-driven position dedicated to helping seniors age safely and independently.
Requirements
- Bachelor's or Master's Degree in Social Work REQUIRED
- Case Management or Related Experience REQUIRED
- Comfortable Conducting Home Visits
- Excellent Communication, Organization, and Documentation Skills
Nice To Haves
- Experience Working with Older Adults Preferred
