Requirements

Registered Nurse of Licensed Clinical Social Worker (LCSW/MSW) or equivalent human services professional
3+ years of experience in geriatric care management, aging services, home care, case management, or related field
Strong knowledge of community resources, healthcare systems, and long-term care options
Valid driver's license and reliable transportation

Nice To Haves

Care Management Certification (CMC) or Aging Life Care Professional® certification
Knowledge of Medicare, Medicaid, and long-term care planning
Experience working with older adults and individuals with chronic illnesses, dementia, or disabilities

Responsibilities

Conduct comprehensive client assessments and develop personalized care plans that support health, independence, and quality of life
Coordinate care and connect clients with healthcare services, community resources, and supportive programs tailored to their needs
Serve as a trusted advocate for clients and families, helping them navigate healthcare decisions and access appropriate services
Collaborate with physicians, healthcare providers, caregivers, and community partners to ensure seamless communication and care coordination
Support clients through care transitions, including hospital discharges and placement planning for rehabilitation, assisted living, or skilled nursing settings
Provide ongoing guidance, education, and support to clients and families while monitoring progress and adjusting care plans as needs evolve
