Requirements
- DDS or DMD from an accredited dental school
- Active Arizona dental license (or ability to obtain prior to employment)
- Current CPR certification
- DEA registration and NPI number (or ability to obtain)

- NHSC Loan Repayment Eligible: This practice is an approved National Health Service Corps (NHSC) site, making eligible providers qualified to receive up to $150,000 in student loan repayment, depending on program eligibility and service commitment.
