As the Functional Family Specialist, you will be responsible for assessing client interactions, formulating interventions based on the identified treatment plan, evaluating the progress of treatment goals, and providing direct care services. The position will be assigned to the 2nd judicial district cases. Specific responsibilities include direct services such as providing a minimum of one-hour weekly, home-based family therapy sessions to clients utilizing the Functional Family Therapy model with families who have adolescents between the ages of 10 and 18. You will handle a caseload of 10 to 15 families, providing services anywhere from eight to 26 weeks, and maintain flexibility in schedule to meet families when they are available. Case management implementation includes facilitating community resource planning, case progress evaluation, permanency planning, participating in discharge planning and documentation, and interfacing with school, medical & psychiatric contacts, referring worker and collaterals. You will also collaborate with stakeholders for case management.
