About The Position

This position supervises, trains, and mentors CARES Care Coordinators in the CARES model serving Family Support Services (FSS) families safe at high or very high risk. The FSS Supervisor ensures the model is delivered with high fidelity and that all contractual and performance measures are adhered to. This position provides oversight of the team's service authorization, Family Team Conferencing, and Wraparound process in a flexible, responsive, culturally competent service delivery system. This position will provide on-call support to the families served and participate in quality improvement processes when indicated.

Requirements

Min 3 years’ Experience delivering CARES Model as a Wraparound practitioner.
Minimum of 3 years’ experience as a wraparound practitioner and/or developing and implementing wraparound strength-based communities of care.
Proven history of collaborating and partnering with collateral and service providers to successfully foster an effective and family-centered team approach.
Supervisory and Management Minimum 3 years: Lead and manage a team of Care Coordinators and will develop performance targets and other deliverables as required while providing ongoing monitoring, continuous quality improvement and support.
Skilled in analytical reasoning and critical thinking.
Assist staff in addressing complex situations with families and problem solving in a creative and flexible environment in which critical thinking is essential.
Valid Driver's License - Required to travel FTC and other meetings.
Strong written and verbal communication skills - essential to document, summarize and articulate data, actions and to effectively build consensus with multiple team and family members.
Strong Project Management Skills are required to lead the start of multiple projects that involve a variety of stakeholders.
BA/BS - bachelor’s degree in human services or equivalent required.
Child Welfare Case Management Certification required.
2+ years Wraparound and staff management experience required.
3+ years child welfare system of care work required.

Nice To Haves

Master’s degree in human services related field preferred.
Child Protection Certification Preferred

Responsibilities

Serve as CARES model SME to lead, train, coach and develop staff to excellence in this model.
Oversight of utilization management of prevention and data tracking to identify areas of opportunity for capacity building and effective utilization of resources and to track data outcomes for the program.
Ensure sufficient use of resources and maximization of alternative funding streams.
Co-facilitate Family Team Conferences and assist with complex cases when indicated.
Promote and provide leadership of strong relationships and consensus building with family members, family partners, providers, and the community.
Conduct utilization reviews, ensure consumer satisfaction, and use of the wraparound fidelity tool.
Ongoing review of care plans to ensure they have clearly defined outcomes.
Plan performance expectations, monitor, and appraise employee's job results and performance to develop, counsel, or discipline staff members.
Fulfill other incidental or related duties as assigned, assisting, and training others, and performing duties of higher rated positions from time to time for developmental purposes.
