About The Position

St. Mary’s Center is a community of hope, healing, and justice dedicated to serving older adults, preschool families, and individuals experiencing homelessness in Oakland and the surrounding Alameda County community. St. Mary’s Center operates transitional housing, a senior center, and preschool and is developing permanent supportive housing as part of a comprehensive, collaborative community development strategy along the San Pablo Avenue corridor. We are proud of an excellent multidisciplinary team delivering high-quality services through which St. Mary’s supports vulnerable populations in aging safely, independently, and with dignity. The Community Outreach & Services (COS) Department helps seniors remain safely housed and socially connected while reducing isolation, improving wellness, and promoting access to community-based supports. COS programming includes congregate meals, wellness activities, fall prevention, computer education, health navigation, case management support, behavioral health education, and Friendly Visiting services. The Friendly Visiting Coordinator is responsible for coordinating and implementing St. Mary’s Center’s Friendly Visiting Program and community wellness initiatives that support older adults, homebound seniors, and socially isolated participants. The position coordinates intern/volunteer engagement, participant scheduling, wellness education, supportive in-home visitation, and linkage to health and social services. This role works collaboratively with staff, caregivers, volunteers, healthcare providers, Alameda County partners, and community organizations to ensure participants receive compassionate, coordinated, and person-centered support.

Requirements

  • Associates degree in Social Work, Human Services, Public Health, Gerontology, Psychology, Sociology, or related field preferred, or three years’ experience in a related field.
  • Minimum of 2 years of experience working with older adults, vulnerable populations, or community-based programs preferred.
  • Strong interpersonal, organizational, and communication skills.
  • Ability to build rapport with older adults, caregivers, volunteers, and community partners.
  • Knowledge of aging-related issues including mobility, social isolation, behavioral health, and chronic disease management.
  • California Driver’s License.

Responsibilities

  • Coordinate Friendly Visiting services for homebound seniors and isolated older adults.
  • Conduct individual client assessments through home visits.
  • Match interns/volunteers with participants based on language, culture, interests, and mobility needs.
  • Schedule and monitor in-home, phone-based, virtual, or facility-based visits.
  • Document participant interactions, referrals, and follow-up activities according to program standards.
  • Coordinate participant wellness checks and identify barriers to safety, mobility, healthcare, or social engagement.
  • Support caregiver respite efforts through interns’ volunteers’ companionship and supportive visits.
  • Onboard and train Friendly Visiting interns/volunteers.
  • Provide ongoing support related to elder engagement, trauma-informed care, and cultural humility.
  • Maintain volunteer schedules, attendance tracking, and service documentation.
  • Conduct outreach and engagement to Medi-Cal eligible seniors and vulnerable populations.
  • Provide health education and preventive wellness information.
  • Participate in data collection and service tracking activities that support reimbursement and reporting efforts.
  • Maintain accurate documentation and reporting aligned with county and state requirements.
  • Promote COS programs through outreach events, presentations, and community engagement activities.
  • Assist with congregate meal services, workshops, and cultural programming.
  • Participate in emergency preparedness and emergency response activities.
  • Collect participant feedback and support program evaluation efforts.
  • Maintain accurate and timely documentation in agency and County systems.
  • Complete assessments, progress notes, referrals, and discharge summaries.
  • Maintain compliance with HIPAA, state, county, city, and agency standards.
  • Track units of service, outcomes, and referrals.
  • Complete 20 hours of Community Health Worker training and 6 hours of continuing education annually.
  • Participate in required quality assurance activities and trainings.
