About The Position

The Foster Care Program Coordinator II is an advancement of Foster Care Program Coordinator I. The position supports members and foster homes that require more therapeutic interventions in Therapeutic, Specialized and CDH foster homes ensuring safety, compliance and stability within a foster home environment. This role is critical in supporting the well-being of children/youth in foster care and ensuring that foster families are equipped to provide a nurturing and safe home.

Requirements

  • Minimum 21 years of age.
  • Able to pass a minimum 5 panel drug test & submit to regular screening.
  • DPS Level I fingerprint clearance (must possess upon hire and maintain throughout employment).
  • Maintain a clear Central Registry Background Check.
  • CPR, First Aid (must possess upon hire and maintain throughout employment).
  • Current, valid Arizona Driver's License and 39-month Motor Vehicle Report and proof of vehicle registration liability insurance to meet insurance requirements.
  • Minimum of 3-years’ experience working in foster care system and/or behavioral health care coordination.
  • BH Level: BHT

Responsibilities

  • Monitor and record behavior problems identified in the home, school and/or community in coordination with the treatment team and foster family.
  • Provide guidance and support to foster parents on effective strategies to address behavior problems.
  • Document progress and response to interventions.
  • Refer foster child and/or family to behavioral health outpatient services in coordination with the treatment team.
  • Report critical incidents within contractual time frames to Supervisor.
  • Document all required documentation (service planning, interventions and incidents) in the EHR.
  • Assess the living environment, safety issues, and ensure adherence to foster care regulations and organization goals during home visits.
  • Document all home visits and contacts in the EHR/OLR/DCS.
  • Train and assist foster parents in providing treatment strategies addressing member concerns/needs.
  • Train and assist foster parents on using the provider portal to upload required documents.
  • Offer training and resources to foster parents, enhancing their skills in providing care and support to foster children.
  • Evaluate foster parent performance and provide constructive feedback.
  • Ensure that all foster homes comply with state and federal regulations regarding safety, health, and welfare standards.
  • Assist foster parents in reporting required progress notes and other documents.
  • Assist in the re-licensure process of foster parents by collecting and/or guiding them through the process.
  • Respond promptly to any safety concerns or incidents, taking necessary actions to protect children in care.
  • Address immediate licensing violations to foster parents.
  • Work closely with the treatment team, therapists, and other stakeholders to ensure a holistic approach to care.
  • Work with Supervisor/Director and Foster Care Compliance Manager assisting in re-licensure and maintenance of training records.
  • Participate in supervision with Supervisor and other team members providing updates on foster homes.
  • Participate in team meetings and maintain professional relationships with internal and external stakeholders.
  • Promote awareness of foster care needs and assist in the recruitment of potential foster families as needed.
  • Perform other duties as assigned and in relation to the foster care program.
