Requirements

  • Bachelor’s degree in social work or a related human service field is required.
  • A minimum of two (2) years of relevant experience is required.

Responsibilities

  • Develop and implement individualized treatment and service plans that support each child’s safety, stability, and well-being.
  • Coordinate medical, educational, behavioral health, and social services to ensure seamless, uninterrupted care.
  • Serve as a central point of communication between foster parents, biological families, service providers, and internal teams.
  • Facilitate and support healthy interactions between children, foster families, and birth families to promote positive outcomes.
  • Monitor and continuously assess each child’s progress, adjusting care plans to meet evolving needs.
  • Maintain accurate, timely documentation and case records in accordance with organizational and regulatory standards.
  • Partner with foster parents to provide guidance, support, and resources that help create safe, nurturing home environments.
  • Collaborate with internal teams and external agencies to ensure coordinated service delivery and compliance with all requirements.
  • Support foster parent recruitment, assessment, and ongoing engagement.
