Foster Care Case Manager
About The Position
This position is responsible for monitoring the placement, treatment, and activities related to foster care assigned caseload and foster parents. The role may work with both Host Home and Foster Care Plus Youth, coaching/counseling individuals/families within their caseload and providing crisis intervention. It coordinates case management and linkages to community programs and services. Case sessions are primarily delivered in the community (client homes, schools, etc.), while case management can occur in an office environment or community. The role may require working evenings and/or weekends to meet client needs and includes on-call coverage.
Requirements
- Bachelor's Degree in a human service-related field
- At least two (2) years of direct care and/or case management experience
- Valid Driver's License and use of personal automobile is required
