About The Position

This position is responsible for monitoring the placement, treatment, and activities related to foster care assigned caseload and foster parents. This role may work with both Host Home and Foster Care Plus Youth. This role coaches/counsels individuals/families within their caseload and provides crisis intervention. The position coordinates case management and linkages to community programs and services. Case sessions are primarily delivered in the community (client homes, schools, etc.), case management can be in an office environment or community. May at times be required to work evenings and/or weekends to meet the needs of the clients. This position requires on-call coverage.

Requirements

• Bachelor's Degree in a human service-related field.
• At least two (2) years of direct care and/or case management experience.
• Valid Driver's License and use of personal automobile is required.

Responsibilities

• Coaching/counseling individuals/families within their caseload.
• Providing crisis intervention to youth in foster care.
• Coordinating case management and linkages to community programs and services.
• Working with both Host Home and Foster Care Plus Youth.
• Working evenings and/or weekends to meet the needs of the clients.
• On-call coverage.
