The Forensic Registered Nurse Navigator is responsible for providing continuity of care by ensuring smooth transitions between care settings. This role involves developing relationships with patients and their multidisciplinary teams to facilitate and navigate subsequent treatment and follow-up. The nurse will complete nursing assessments to identify individual patient needs, collaborate with multidisciplinary teams to develop interdisciplinary plans of care, and support patients and families by providing education, addressing psychosocial needs, advocating, and identifying barriers to care. The navigator will guide patients and family members throughout the diagnosis, treatment, and follow-up process across the continuum of care. Additionally, this role provides expert clinical advice to the multidisciplinary team in designing collaborative teaching plans, planning patient/family education, and discharge planning, including wellness and health maintenance within the designated specialty. The position also involves coordinating timely, seamless, evidence-based care across the care continuum to achieve optimal patient outcomes, evaluating and selecting patient education materials, and providing education and outreach programs to patients, Advocate staff, and community members. The nurse will also assist leadership in developing program vision, goals, and objectives, and in the development, implementation, and maintenance of quality programs focused on best practice patient care outcomes, quality of life, and enhanced patient loyalty. The incumbent must demonstrate knowledge and skills appropriate for the age of the patients served, understanding principles of growth and development and assessing data to identify age-specific needs.
