About The Position

EAC Network, a not-for-profit social service agency, is seeking a full-time Forensic Case Manager for its Manhattan/ Brooklyn Mental Health Diversion Program. This role involves providing screening, service planning, case management, and court reporting for individuals with mental health and/or co-occurring substance use needs who are involved in the criminal legal system. The Forensic Case Manager supports diversion efforts by coordinating treatment, monitoring compliance, and facilitating communication between clinical providers and the court.

Requirements

  • Associate’s or Bachelor’s degree in criminal justice, social work, counseling, forensic psychology, psychology, sociology, or criminology and/or a significant combination of education and experience in either mental health, substance abuse or criminal justice required.
  • Good communication skills, both oral and written.
  • Ability to work cooperatively with treatment and criminal justice systems.
  • Must be proficient in computer applications or other automated systems such as spreadsheets, Microsoft Office applications, calendar, and email and/or database software necessary to perform work assignments.

Responsibilities

  • Serve as liaison to courts, correctional facilities, and community-based treatment providers, supporting diversion and Alternatives to Incarceration (ATI) efforts.
  • Conduct screenings and assist in identifying appropriate treatment and service needs for individuals presenting with mental health and/or substance use concerns.
  • In collaboration with supervision, facilitate referrals to mental health, substance use, residential, and supportive services, and coordinate client placement as appropriate.
  • Maintain an active caseload and monitor client compliance with court mandates and treatment participation through regular contact, case conferencing, and review of provider reports.
  • Prepare timely written and verbal court reports detailing client progress, compliance, and any clinical or behavioral concerns.
  • Maintain accurate, timely, and audit-ready documentation, including progress notes, case records, and all required reporting in accordance with program and regulatory standards.
  • Assist clients in accessing benefits and entitlements, including public assistance, Medicaid, and housing resources.
  • Coordinate with treatment providers, court stakeholders, and collateral contacts to support continuity of care and successful program engagement.
  • Conduct field work as needed, including visits to courts, treatment programs, and correctional settings.
  • Conduct toxicology testing as required by program protocols.
  • Identify and recommend supportive services to address client needs, including housing, vocational, and social supports.
