About The Position

The Foregut and Bariatric Program Coordinator serves as the primary clinical and program coordinator for patients participating in medical weight management, metabolic, bariatric, and foregut services, including evaluation, treatment, surgery, and long-term follow-up. This role provides comprehensive patient education, care coordination, navigation, and clinical support throughout the continuum of care while promoting optimal patient outcomes and adherence to evidence-based practice. The coordinator collaborates closely with surgeons, advanced providers, dietitians, behavioral health professionals, primary care providers, and ancillary staff to ensure high-quality, patient-centered care. The coordinator facilitates multidisciplinary evaluations, monitors patient progression through program and insurance requirements, supports long-term follow-up, and assists with quality improvement, outcomes tracking, and accreditation activities.

Requirements

  • Graduate of an accredited school of registered nursing.
  • Current Minnesota Registered Nurse license.
  • Current CPR certification.
  • Knowledge of clinical nursing principles, patient assessment, care coordination, chronic disease management, and patient education.
  • Understanding of obesity medicine, metabolic health, bariatric and foregut conditions, digestive health disorders, and related treatment modalities.
  • Ability to coordinate care across a multidisciplinary team and support patients throughout the continuum of care.
  • Strong organizational, time management, and project coordination skills with the ability to manage multiple priorities and program initiatives.
  • Knowledge of quality improvement principles, outcomes tracking, accreditation standards, and data management practices.
  • Strong critical thinking, clinical judgement, problem-solving, and decision-making skills.
  • Excellent verbal, written, and interpersonal communication skills with the ability to effectively engage patients, providers, staff, and community partners.
  • Proficiency in electronic medical records, standard computer applications, and clinical documentation systems.
  • Ability to work independently while fostering collaborative relationships and contributing to a team-orientated environment.
  • Commitment to patient-centered care, confidentiality, professionalism, and continuous improvement.

Nice To Haves

  • Bachelor of Science in Nursing (BSN) preferred.
  • Minimum of three (3) years of clinical nursing experience.
  • Experience in surgery, gastroenterology, bariatrics, weight management, digestive health, care coordination, case management, quality improvement, or program preferred.
