Oklahoma City Indian Clinic (OKCIC) is a non-profit corporation that provides culturally sensitive health care to the American Indian population. OKCIC services include not only basic medical care but also dental, optometry, behavioral health, fitness, nutrition, and other family programs. The Oklahoma City Indian Clinic (OKCIC) is seeking a compassionate and resourceful Food Resource & Social Services Specialist to join our Public Health team. This position plays a vital role in connecting patients to essential community resources and programs that support their physical, mental, and social well-being, with a strong focus on addressing food insecurity through our in-house Food Resource Center. As a Food Resource & Social Services Specialist, you will coordinate care with healthcare providers, assess patient needs, and assist individuals in accessing a wide range of services — including food and nutrition support, housing, transportation, insurance, and other community resources. This role is instrumental in helping patients navigate barriers to care while ensuring access to nutritious food and supportive services.

Requirements

  • Must align with OKCIC vision, mission, and core values.
  • Bachelor’s degree in Sociology, Public Health, Social Work, or related field preferred
  • 2–5 years of experience in social services, community health, nonprofit programs, or food resource/pantry operations
  • Experience working with Native populations preferred
  • Experience with community outreach, resource coordination, or program support preferred
  • Familiarity with public assistance programs (Medicaid, SNAP, Social Security, etc.) preferred
  • Strong organizational, communication, and multitasking skills
  • Crisis management experience preferred
  • Valid Oklahoma driver’s license with acceptable driving record
  • Acceptable driving record (no more than 2 moving violations in the last 2 years).
  • Must be eligible for OKCIC driver's insurance policy.
  • Current CPR is required or must be obtained within 6 months of hire.

Responsibilities

  • Provides Social Services to patients in assessing needs and determining eligibility for specific programs and services.
  • Coordinates delivery of care with clients and other health care providers, assuring appropriate use of resources.
  • Provides information and/or referrals to appropriate agencies to meet the patient's needs.
  • Provides assistance to personnel to ensure an efficient pathway to facilitate quality, cost-effective care in the least restrictive environment.
  • Assistance and referrals may include: tribal social service, Medicare, Medicaid, private insurance, long-term care, skilled nursing care, hospice, transportation, home health needs, OKDHS, IHS purchased and referred care, other IHS facilities, social security, medication, nutrition, utilities, housing, and/or other resources as needed.
  • Meets with staff regarding social work services, referrals, and community resource acquisition.
  • Verifies benefits coverage with the Benefits Coordinator.
  • Documents treatment plans and progress in the Electronic Health Record.
  • Participates in Social Services-related committees, i.e., Warm & Fuzzy and Elder Community.
  • This includes, but is not limited to, community events and outreach.
  • Responds to performance measure screenings (depression, alcohol abuse, domestic violence, etc.) as needed.
  • Make home visits as required.
  • Prepare monthly reports as requested.
  • Works with Leadership in maintaining Policy and Procedures for Case Management.
  • Coordinates with community governments, health organizations, non-profits, and other community partners.
  • Prevents, addresses, and solves social health problems.
  • Coordinates with third-party social services personnel housed through various partnerships, as directed, including but not limited to the in-house Food Resource Center.
