About The Position

Change Grow Live is a charity dedicated to helping people change the direction of their lives, grow as individuals, and live life to its full potential. They are looking for an enthusiastic and friendly person to join their team as a Floating Support Worker. This role involves working with clients in structured treatment who are entering or are on a path to enter a housing crisis. The position focuses on supporting vulnerable individuals with experiences of cuckooing, unstable housing, learning disabilities, and/or brain injuries. The Floating Support Worker will hold a caseload of service users currently in drug and/or alcohol treatment and will also support colleagues in developing their skills. The role is based in Nottinghamshire, requiring travel throughout the county.

Requirements

  • An understanding and knowledge of the harmful effects associated with drug and alcohol use in relation to health, social welfare, housing, employment, and personal relationships.
  • Minimum of 12 months of experience facilitating successful tenancy sustainment and working to achieve positive community connections.
  • Experience engaging with a wide variety of individuals with diverse backgrounds and needs.
  • The ability to work with and manage challenging behavior.
  • Must have the right to work in the UK at the time of application. This role is not eligible for visa sponsorship.

Responsibilities

  • Establish effective communication and referral pathways for individuals at risk of homelessness and in contact with other health and care agencies.
  • Provide wrap-around support in emergency/temporary/supported accommodations across the area.
  • Prioritize caseload based on crisis intervention and severity of needs.
  • Attend relevant multi-disciplinary team meetings to support links to health and social care services, peer support networks, community assets, and mutual aid.
  • Work closely with the local authority, Health & Social Care sector, partners, and any provision linked to preventing homelessness and improving joint working.
  • Increase and enhance partnership working with primary care, mental health services, housing, and other voluntary organizations supporting individuals at risk of losing tenancies.
  • Provide wrap-around support on budgeting, education, training, and employment while continuing to support engagement in structured services.
