At CSR, our Behavioral Health Department (BHD) offers a comprehensive array of high quality, culturally-sensitive mental health services that provide children and families with the services they need for success at home, in school, and in the community. The Family-Based Intensive Treatment (FIT) Care Coordinator provides intensive, community-based behavioral health services to children and youth under the age of 21 with Serious Emotional Disturbance (SED) and their families. This role is responsible for providing therapeutic support and assisting with care management activities to address the behavioral health needs of youth and their families. FIT Care Coordinators work closely with clinicians, Family Partners, and community resources to help families navigate systems of care, access services, and achieve treatment goals in a timely and effective manner. This role also focuses on building coping skills, supporting daily functioning, and assisting families in achieving treatment goals within home and community settings. This position directly supports CSR’s mission by empowering youth and families to thrive through culturally responsive, trauma-informed, and community-based care. The FIT Care Coordinator plays a vital role in bridging clinical goals with real-world practice to strengthen families’ capacity for resilience and stability.

Requirements

  • Bachelor’s degree in a human services field (preferred) OR High School Diploma/GED with at least 3 years of experience working with children, youth, or families.
  • Experience providing direct support to children/youth with emotional or behavioral health needs is strongly preferred.
  • Ability to engage and build trust with children, youth, and families.
  • Skills in coaching, mentoring, and reinforcing positive behaviors.
  • Strong communication, teamwork, and problem-solving abilities.
  • Knowledge of trauma-informed and culturally responsive approaches.
  • Strong organizational and documentation skills.
  • Must have access to reliable transportation, valid driver’s license, and insurance.

Responsibilities

  • Facilitate and participate in care coordination activities, including phone calls, in-person meetings, and virtual meetings with collateral providers, schools, state agencies, Youth Mobile Crisis Intervention providers, physical health practitioners, and other individuals that may impact the youth’s treatment.
  • Provide skills coaching and support to youth and families in home, school, and community settings.
  • Assist families in practicing coping, communication, and problem-solving strategies identified in the treatment plan.
  • Work collaboratively with the FIT Clinician and Family Partner to ensure treatment goals are reinforced consistently across environments.
  • Participate in family sessions, treatment planning meetings, and team check-ins.
  • Provide crisis support and assist in implementing safety plans developed by the FIT Clinician.
  • Monitor and document all services provided, including progress toward treatment goals, in the youth's service record
  • Serve as a role model for positive social skills, healthy routines, and resilience-building strategies.
  • Maintain accurate and timely documentation in compliance with agency, CBHI, and MassHealth standards.
  • Perform other related duties as assigned.
