About The Position

We are seeking a warm, organized, and detail-oriented First Impressions Coordinator to join our client focused care team. This person will play a key role in ensuring our clients and their families have a seamless, personalized, and consistently positive experience with Coastal Care Partners.

Excellent interpersonal and communication skills—compassionate, patient, and a great listener
Strong organizational skills and attention to detail
Experience in customer service, home care, healthcare, or a related field
Ability to juggle multiple tasks and stay calm under pressure
A team player who thrives in a fast-paced, people-focused environment
Comfortable with scheduling tools, databases, and phone-based communication

Nice To Haves

Previous experience working in a home care, medical office, or senior care setting
Knowledge of scheduling systems (e.g., AxisCare/ClearCare/WellSky/Athena or similar platforms)
A background in nursing, care coordination, or case management

Responsibilities

Serve as a primary point of contact for our medical and home care clients.
Proactively check in with clients to ensure satisfaction and quickly address any concerns
Manage care transitions and updates, including changes in client condition or care plan
Maintain accurate client records and communication notes in multiple systems
Support the onboarding of new clients and help create personalized care experiences
Collaborate closely with medical providers, leadership, caregivers and internal teams to ensure quality and consistency
