About The Position

The Field Supervisor is responsible for planning, coordinating, and documenting non-skilled supervisory visits for the Philadelphia and surrounding county areas of clients following Home Care policies and procedures. Field Supervisors assure compliance with local, state, and federal laws, Medicare regulations, and established company policies and procedures. The field supervisor is responsible for the direct supervision of the personal care assistant (PCA). The field supervisor performs field evaluations and serves as a mentor and support for field staff.

  • Ability to work as part of a team and maintain positive working relationships.
  • High School Diploma
  • At least one (1) year of previous experience in health care.
  • Proficient with Microsoft Office Suite or related software.
  • Must have the use of an automobile in good working condition, a valid driver's license, and automobile insurance compliant with company policies.
  • Ability to move, transport, or position up to 50 pounds
  • Ability to move or traverse about in offices and/or client homes, including ascending and descending stairs.
  • Ability to communicate effectively and clearly with others to exchange information.
  • A valid driver�s license and reliable personal vehicle are required for this position.

Nice To Haves

  • CNA or HHA certification preferred.
  • Associate's degree or equivalent industry experience preferred

Responsibilities

  • Assesses new client needs upon admission and develops the initial care plan.
  • Confirms schedule for care and follows up with clients to ensure that quality care is received.
  • Performs in-person/virtual client reassessments and notifies Team Lead of any changes in the client's condition that may necessitate changes to the established care plan.
  • Performs direct observation of PCAs for annual performance evaluation and provisional monitoring of newly hired staff.
  • Assists in closing client care gaps and conducts unannounced home visits as necessary.
  • Communicates client problems, concerns, changes, and complaints to appropriate staff, and prepares reports of incidents that could result in legal action.
  • Maintains and develops positive relationships with existing and prospective clients, demonstrating excellent customer service and setting an example for other staff.
  • Maintains daily schedule in HHAX and accurately completes visits in a timely manner.
  • Participates in staff meetings, agency sponsored in-services, and team meetings, as directed.
  • Performs other duties and assists with special projects as requested.
