About The Position

Optum Home & Community Care, part of the UnitedHealth Group family of businesses, is creating something new in health care. They are uniting industry-leading solutions to build an integrated care model that holistically addresses an individual’s physical, mental and social needs – helping patients access and navigate care anytime and anywhere. As a team member of the Senior Community Care (SCC) product, this role involves working with a team to provide care to patients at home in a nursing home, assisted living for senior housing. This life-changing work adds a layer of support to improve access to care. The position is part of the primary care team, collaborating with all interdisciplinary team members and supporting multiple providers. The RN is a flexible team member who works alongside nurse practitioners (NPs) and physician assistants (PAs). This is a field based position traveling to facilities in Butler and Lawrence Counties, PA.

Requirements

  • Associate degree or higher in Nursing (RN)
  • Active unrestricted Registered Nurse license in your state of residence or ability to obtain within 60 days of hire
  • Certified in Basic Life Support
  • 1+ years of experience working as a RN
  • Driver’s license and access to reliable transportation that will enable you to travel up to 100% to visit clients and/or patient sites within a designated area

Nice To Haves

  • BSN degree
  • Experience working in Long Term Care and/or Assisted Living

Responsibilities

  • Responsible for collaborating with all members of the interdisciplinary care team around changes in condition and/or care coordination
  • Triage member needs to the appropriate care team member and level of care
  • Assist with transitions in care: outpatient and inpatient
  • Assist in assuring complete and accurate documentation, medication reconciliation, nursing rounds, and risk assessments
