About The Position

We are looking for a Field Nurse for our Licensed Home Care (LHCSA) team in Albany, NY. This per-diem position offers flexible hours and requires regular travel to visit patients in their homes across Albany, NY, and nearby areas. Compensation is based on each completed assessment or visit. The Field Registered Nurse is an integral team member of the FreedomCare Licensed Home Care team. They assess our patients' current service needs and reassess needs for service upon significant change in health status.

Requirements

  • Bachelor's Degree (BSN) and current NYS License or compact License that is in good standing (OR) associate's degree in nursing with relevant homecare clinical experience.
  • Minimum of 1-2 years of Nursing experience with a concentration in the homecare agency space.
  • Experience in geriatric care or working with individuals with disabilities.
  • Ability to travel within the Albany, NY and surrounding areas, as needed.
  • Current knowledge of state and federal home health regulations.
  • Strong interpersonal and communication skills.
  • Proficient in Microsoft Office Suite (Word, Excel, Outlook, etc.).
  • Excellent verbal and written communication, organizational and time management skills.
  • Required valid driver’s license and reliable transportation.

Nice To Haves

  • Master's degree in nursing.
  • Experience in Home Care, Geriatrics and Case Management; field experience assessing patient care.
  • 3+ years of homecare agency experience.
  • Previous experience in a non-skilled home care setting.
  • Bilingual in one of the following languages: Spanish, Russian, Chinese, Korean and Bengali.
  • Ability to work collaboratively with caregivers and families.
  • Proficient in electronic health record (EHR) systems.
  • Strong experience using a Uniform Assessment System (UAS).

Responsibilities

  • Conduct initial assessments of clients to determine care needs and develop individualized care plans.
  • Train and supervise non-skilled caregivers in providing personal care, companionship, and assistance with daily living activities.
  • Monitor clients’ overall well-being and report any changes to the appropriate healthcare professionals.
  • Educate clients and their families on effective care strategies, safety measures, and daily living skills.
  • Ensure compliance with agency policies, state regulations, and best practices in non-skilled care.
  • Maintain accurate documentation of client care and progress, while ensuring confidentiality.
  • Act as a liaison between clients, families, and caregivers to facilitate communication and enhance the quality of care.
  • Participate in ongoing training and professional development to stay current with best practices in home care.
