About The Position

True Care is seeking multiple Per Diem Fee-for-Service Registered Nurses (RNs) to join their clinical team. This field-based position involves conducting in-home patient assessments in New York City and Westchester County, ensuring patients receive high-quality, compassionate care. The role specifically requires nurses with evening and weekend availability to support after-hours and weekend assessment coverage. It is ideal for experienced RNs seeking flexibility, autonomy, and the chance to make a significant impact in home care.

Requirements

• Active New York State Registered Nurse (RN) license in good standing.
• Minimum one (1) year of nursing experience preferred.
• Ability to work independently and effectively manage a field-based schedule.
• Willingness to travel throughout New York City's five boroughs and Westchester County.
• Availability to work evening and weekend shifts is required.
• Excellent interpersonal, communication, and organizational skills.

Nice To Haves

• Home care experience is a plus.
• Bilingual in Spanish, Creole, or other languages is highly preferred.

Responsibilities

• Conduct comprehensive in-home patient assessments, including Start of Care (SOC) and other clinical assessments as assigned.
• Accurately evaluate patients' medical conditions, functional status, and home care needs.
• Complete all required clinical documentation in a timely and accurate manner.
• Develop and update patient plans of care in accordance with agency policies and regulatory requirements.
• Collaborate with physicians, clinical leadership, Home Health Aides (HHAs), Personal Care Aides (PCAs), and the interdisciplinary care team to coordinate patient services.
• Educate patients and caregivers on disease management, medications, safety, and home care services.
• Report significant changes in patient condition to the appropriate clinical personnel.
• Ensure compliance with all agency, state, federal, and Department of Health regulations.
• Deliver compassionate, patient-centered care while maintaining professionalism during every patient interaction.
