About The Position

The Case Manager for Home Health provides nursing care in accordance with the client's plan of care, and includes comprehensive health and psycho-social evaluation, monitoring of client's condition, health promotion and prevention, coordination of services, teaching and training activities and provision of direct nursing care and other duties as deemed necessary.

Requirements

  • Graduate of accredited nursing program.
  • 2 years of RN experience with one (1) year experience with adult IV expertise required.
  • (RN) REGISTERED NURSE by Board of Nurse Examiners for the State of Texas (or BNE Compact State).
  • (DRIVERS) DRIVERS LICENSE Valid Texas driver's license, proof of driver's insurance.
  • (BLS) BASIC LIFE SUPPORT accredited course by the American Heart Association (AHA) or American Red Cross (ARC)

Nice To Haves

  • Bachelor's Degree - BSN
  • Other RN experience including high tech and home health expertise preferred.
  • Prefer knowledge of InterQual and TMF care criteria.

Responsibilities

  • Provides direct nursing care, including patient observation and delivery of prescribed treatments.
  • Conducts initial and ongoing comprehensive assessments of home health patient needs, including OASIS assessments at appropriate time points.
  • Evaluates outcomes of patient care and identifies discharge planning needs.
  • Consults with other disciplines as required.
  • Adjusts nursing care processes as necessary.
  • Maintains patient records and charts to ensure accurate record of treatment, including med profile, care plan, communication notes, etc.
  • Provides instructions to patients/families for use of products and services utilizing nursing process to address learning needs.
  • Prepares, administers, monitors, and discontinues IV fluids.
  • Accesses IV delivery system (ie. peripheral IV start, implant cath, broviac, etc.).
  • Administers medications and treatments as prescribed by the physician.
  • Performs phlebotomy, laboratory specimen collection and point of care testing.
  • Responsible for overall delivery of quality patient care that meets or exceeds established standards and provides assistance in activities to meet state and/or federal regulatory guidelines.
  • Maintains safe, comfortable, and therapeutic environment for patients/families in accordance with established standards.
  • Participates in Continuous Quality Improvement (CQI) Program.
  • Provides supervisory visits for Home Health Aides and LVNs as required by state and federal regulations.
  • Responsible for participating in on-call rotation and emergency call according to agency policy.
  • Performs other duties as assigned.
