About The Position

Care First is seeking a Field Outreach Representative to join our team in The Bronx. This role is responsible for engaging community members, generating referrals, and connecting eligible individuals to our programs and services. The Field Outreach Representative serves as the face of the organization in the community, building relationships with local partners and educating prospective participants about available resources. The ideal candidate is outgoing, self-motivated, and passionate about serving underserved populations. They enjoy working in the field, developing community partnerships, and helping individuals access healthcare and social support services.

Requirements

  • 1–3 years of experience in community outreach, healthcare outreach, sales, customer engagement, social services, public health, or a related field.
  • Strong communication and interpersonal skills with the ability to connect with diverse populations.
  • Experience building professional relationships and community partnerships.
  • Ability to work independently and manage a flexible schedule.
  • Comfortable working in the field, attending events, and conducting face-to-face outreach activities.
  • Strong organizational and time-management skills.
  • Experience using CRM systems and Microsoft Office applications preferred.
  • High school diploma or equivalent required; associate's or bachelor's degree preferred.
  • Valid driver's license and reliable transportation may be required.

Responsibilities

  • Conduct Community Outreach: Represent Care First at community events, health fairs, resource fairs, and partner locations. Conduct face-to-face outreach activities to increase awareness of available programs and services. Distribute educational materials and provide information to prospective participants and community stakeholders.
  • Generate Referrals and Leads: Identify and engage individuals who may benefit from organizational programs and services. Collect preliminary information and refer interested individuals to the Enrollment Team for eligibility screening and intake. Maintain a consistent pipeline of qualified referrals through proactive outreach efforts.
  • Build Community Partnerships: Develop and maintain relationships with healthcare providers, community-based organizations, shelters, faith-based organizations, senior centers, and other referral sources. Schedule and conduct presentations to community groups and partner organizations. Strengthen referral networks to support enrollment goals and community impact.
  • Educate and Advocate: Educate community members about Medicaid-related programs, social determinants of health services, and available community resources. Address questions and concerns while providing accurate information about organizational offerings. Serve as a trusted resource and advocate for individuals seeking support services.
  • Track Outreach Activities: Document outreach efforts, referrals, community contacts, and event participation in CRM and tracking systems. Maintain accurate records and submit activity reports as required. Monitor outreach performance and contribute to achieving monthly and quarterly goals.
  • Collaborate with Internal Teams: Partner closely with Enrollment Specialists to ensure a seamless transition of referred individuals into the enrollment process. Communicate referral trends, community needs, and partnership opportunities to leadership. Support special projects and outreach initiatives as assigned.
