About The Position

This role involves providing safe, quality, therapeutic, and age-appropriate nursing care using evidence-based practice. The Field Nurse utilizes clinical expertise in assessment, clinical decision-making, clinical practice, and patient education. They coordinate care with the interdisciplinary team to promote optimal patient outcomes and satisfaction. This position is for per-visit Field Nurse roles in Brooklyn, Nassau/Garden City, and Eastern Long Island regions.

Requirements

  • Current Registered Professional Nurse in New York State
  • Bachelor's Degree in Nursing
  • At least one year of home care/home health experience
  • Basic Life Support Certification through the American Heart Association
  • Fluent written and spoken English language skills
  • Excellent communication (both oral and written), interpersonal, customer service skills
  • Critical thinking skills
  • Working knowledge of computers
  • Qualified candidates must be able to effectively communicate with all levels of the organization.

Responsibilities

  • Completes a comprehensive nursing assessment including but not limited to physical, functional, psychosocial, emotional, cognitive, sexual, cultural, age related, environmental, spiritual, transpersonal, and economic factors in a systematic and ongoing process while honoring the uniqueness of the patient.
  • Derives the diagnoses and needs from the assessment data, validates the diagnoses, and collaborates with the patient, family, caregivers, and physician to develop an individualized plan care to attain expected quality outcomes.
  • Responsible for the coordination of care by interacting and collaborating with other members of the healthcare team to optimize patient outcomes, patient satisfaction, and health care goals.
  • Delivers direct nursing care, ongoing assessment, teaching and training to patients and caregivers that preserves and protects patient autonomy, dignity, rights, values, and benefits using evidence-based nursing practice.
  • Identifies deficits and barriers to effective knowledge and understanding of education provided to patients. Employs strategies to optimize patient engagement. Demonstrates caring behaviors toward patients, family, and other caregivers.
  • Complete and maintain accurate, complete, and timely documentation reflecting the assessment, examination, diagnosis, prognosis, plan of care, education intervention, and evaluation according to Federal, Department of Health and The Joint Commission requirements.
  • Assist Agency leadership with achieving corporate goals, improving quality of patient care outcomes and patient satisfaction. Participate in Agency quality assessment and performance improvement initiatives.
