About The Position

A Path of Care Home Health in Yukon, OK is seeking a compassionate and skilled On-Call Weekend PRN Field Nurse (LPN) to join our team. This role includes providing high-quality patient care in the home setting, as well as diabetic visits in facility settings, supporting patients with blood glucose monitoring, education, and ongoing management as ordered. Working closely with physicians, we create personalized care plans carried out by our dedicated nurses, therapists, and aides. Requirements

  • Active LPN license in the state of Oklahoma
  • CPR certification (or ability to obtain within 60 days)
  • Minimum 1 year of nursing experience
  • Valid driver's license and reliable transportation
  • Ability to travel long distances and adapt to schedule changes

Responsibilities

  • Performing skilled nursing tasks such as injections, wound care, and equipment preparation
  • Teaching patients' self‑care techniques to promote independence
  • Communicating with RNs and case managers to update care plans and clarify instructions
  • Assisting RNs or physicians with specialized procedures
  • Following all oral and written care instructions
  • Practicing infection control and universal precautions
  • Reporting suspected abuse in accordance with state and federal guidelines
  • Maintaining punctuality, accurate documentation, and required driving records
