About The Position

A Path of Care Home Health offers skilled care and support to our patients so they can live safely and independently in their own homes. We work directly with doctors to develop individualized care plans that our nurses, therapists, and aids follow to ensure our patients receive the best care possible to heal. Requirements

Active nursing license in the state of Oklahoma
CPR certification or the ability to obtain one within 60 days of being hired
1+ years of nursing experience
Valid driver's license and reliable transportation
Ability to travel long distances and adapt to schedule changes
Basic knowledge about computers
Excellent verbal and written communication skills
Ability to balance multiple tasks while demonstrating good time management
Ability to easily adapt to new situations
Ability to work well under pressure
Exceptional problem-solving skills
Ability to maintain a compassionate and empathetic manner toward others

Experience working in home health, critical care, or an emergency room

Responsibilities

Visiting patients in their homes to ensure they receive high-quality care that meets their specific needs.
Providing hands-on nursing skills, including injections and wound care.
Ensuring equipment and materials are prepared correctly.
Teaching patients appropriate self-care techniques.
Communicating with RNs and case managers to provide updates on each patient and clarify information when instructions are unclear.
Aiding RNs or physicians in performing specialized procedures and duties.
Carrying out oral or written instructions.
Practicing infection control and universal precautions to ensure patient safety.
Reporting any forms of abuse and taking appropriate action according to state and federal guidelines.
Maintaining regular attendance to complete day-to-day tasks.
Submitting documentation regarding clinical notes and driving records.
